Description
Provides quality patient-oriented pharmaceutical care which includes providing drug information and consultation to physicians and other health care personnel, receiving medication orders from health care providers, reviewing each for appropriate therapy, managing the dispensing of medications to the patient care areas and monitoring patient response to drug therapy for a specific patient population.
Provides educational programs and inservices
- Acts as primary preceptor for residents in practice area by acting as a role model for residents by exhibiting a commitment to and leadership in the profession; applying the four preceptor roles of direct instruction, modeling, coaching, and facilitating; managing daily activities to ensure rotation objectives are met; and providing effective and timely feedback and evaluation.
- Seeks opportunities to educate other team members.
- Supports institutional and departmental programs.
- Effective educational techniques are utilized.
- All communication is addressed at the appropriate level for the audience.
Documents activities in the medical record
- Appropriate activities are selected for documentation.
- Documentation is completed in time to be useful.
- Hospital policies and procedures are followed including that entries are signed, dated, timed, legible, and concise.
- Activities and recommended plans are clearly presented.
Pharmacotherapy management
- Drug therapy problems are identified.
- Drug therapy regimens/recommendations are designed to reflect best evidence.
- Appropriate monitoring plans are developed and implemented.
- Recommendations are clearly and professionally presented.
- Attempts to review/validate orders in a timely manner and is consistent with established guidelines.
- Responses to consults are timely and occur in accordance with established guidelines.
- Responses to consults are appropriate, complete, and presented in a professional manner.
- Monitoring plans are clearly communicated.
- When participating in the management of medical emergencies, responses are timely, recommendations are appropriate, and additional support is provided as warranted.
Requirements
Pharm.D. degree from ACPE accredited College of Pharmacy, ASHP accredited residency required, and Tennessee licensure.
