Vineyard Post Acute is seeking Occupational Therapists (OTs) to join our dedicated rehabilitation team. Job Responsibilities
- Evaluate and develop individualized treatment plans for residents
- Implement evidence-based therapy interventions to improve daily living skills
- Work collaboratively with an interdisciplinary team to enhance resident outcomes
- Educate residents and families on adaptive techniques and rehabilitation goals
- Maintain accurate documentation in compliance with regulatory standards
Qualifications
- Active Occupational Therapist (OT) license in California
- Passion for helping patients regain function and independence
- Strong teamwork and communication skills
- SNF experience preferred, but new graduates are encouraged to apply
