• Complete and coordinate the Minimum Data Set (MDS) process in compliance with state, federal, and facility guidelines
• Accurately assess and document resident needs and conditions
• Collaborate with the interdisciplinary team to ensure care plans reflect resident goals and clinical requirements
• Monitor quality measures and ensure accurate reimbursement through proper coding and documentation
• Assist in training staff on MDS-related protocols and updates
• Support the Director of Nursing and leadership team in driving quality outcomes and regulatory compliance

• Active California LVN or RN license required
• Minimum of 1-2 years of MDS experience in a skilled nursing facility preferred
• Strong knowledge of PDPM and care planning process
• Excellent clinical judgment, organizational skills, and attention to detail
• Strong communication and teamwork skills
• Passion for improving patient outcomes and ensuring accurate documentation
