College of Medicine will either be hiring for a Community Health Worker I or II. If not a certified CHW, will need to attain CHW certification within 1 year of hire.
**This position is part of the CHW program at the College of Medicine**
The position will work with community members from the Third Ward and East End as part of the Household Centered Care Program at the Tilman J. Fertitta Family College of Medicine, Office of Community Health. CHWs connect community members to medical and behavioral health services, food, and other social needs. The work will be conducted with an interprofessional team of medical, nursing, and social work students. Candidates for this position must be able to conduct home or community visits in person and must have access to dependable transportation. Mileage for work-related travel is reimbursed. This position is in-office from 8 AM to 5 PM, with evening and weekend events. Advance notice will be given for evening and weekend assignments.
Preferred Experience: Case management and resource navigation experience preferred. Knowledge and experience with Third Ward preferred.
Community Health Worker I
Hourly Salary Range: 16.63-18.73
Helps move program efforts forward with the UH College of Medicine Community Health Worker Initiative by providing outreach, navigation and eligibility assistance for participants of various programs.
1. Serves as point of contact between community/neighborhood and the UH College of Medicine Community Health Worker Initiative.
2. Establishes rapport with community members and families participating in the UH CHW initiatives.
3. Recruits program participants for Household Centered Care programs.
4. Conducts home/site visits to identify needs for services and provide information and referrals to diverse resources.
5. Maintains case record documents providing thorough documentation in a timely manner.
6. Partners with CHWs and staff members to assist in contacting community members participating in UH initiatives.
7. Adheres to policies and procedures of the organization and UH College of Medicine Community Health Worker Initiative.
8. Maintains partnerships with community-based organizations (local schools/ school districts or health and community non-profits or stakeholders in the CHWs community/neighborhood.
9. Performs other job-related duties as required.
Community Health Worker II
Hourly Salary Range: 18.32-20.60
Provides guidance to lower-level Community Health Workers (CHWs) within the Tilman J. Fertitta Family College of Medicine (COM) on care coordination duties as well as conducting navigation and eligibility assistance for participants of various programs. Performs outreach, enrollment activities and health education initiatives. Helps move program efforts forward, across programs for the UH College of Medicine Community Health.
1. Organizes workload for lower level CHW regarding outreach and care coordination.
2. Maintains case record documents and information in an automated system as it relates to UH College of Medicine Community Health Worker Initiative.
3. Ensures identification of needs with Household Centered Care participants and other programs.
4. Provides information as needed to staff members regarding health needs or special cases in the community.
5. Delivers health education classes.
6. Assists other CHWs in helping families create goals and care plans of action to engage healthy behaviors.
7. Identifies patient compliance barriers, still ensuring quality of care coordination delivery.
8. Assists in developing, organizing and coordinating continuing education classes, training and opportunities for team members, CHWs and other professionals in the community
9. Facilitates the coordination of undergraduates, medical students or other interprofessional students, inclusion in community activities.
10. Prepares reports of tasks and activities and keeps complete records of outreach events, activities and contacts along with people reached.
11. Provides updated inventory of health education materials and community resources in the area.
12. Prepares, organizes and executes care coordination initiatives for team members.
13. Assists with the implementation and on-going monitoring of care coordination delivery.
14. Facilitates hub meetings for students, COM faculty and staff to identify high/low risk participants for outreach and care coordination.
15. Maintains partnerships with community-based organizations (local schools/ school districts or health and community non-profits or stakeholders in the CHWs community/neighborhood).
16. Performs other job-related duties as required.
MQ: Education: Requires a basic knowledge of grammar, spelling, punctuation and simple mathematical functions like percentages, ratios, etc. as might normally be acquired through attainment of a high school diploma or GED. Experience: Requires a minimum of one (1) year of directly job-related experience. Certification/Licensing: Texas State Community Health Worker Certification.
